Confidentiality in child and adolescent mental health is a complex and often challenging matter. Not only do young people frequently present to services in situations of risk, they often prefer to keep information confidential from parents and/or other professionals. This article explores confidentiality in the context of child and adolescent mental health services (CAMHS), particularly when the clinician is having to make decisions on whether to maintain or to breach it. The 2014 English Special Educational Needs and Disability (SEND) Code of Practice, a policy framework for delivery of a range of services including Schools Mental Health Services (SMHS), is considered in the context of international research on the delivery of mental health services in schools. Effective interventions have been reported in poor and middle-income countries, whereas there is less information about routine services in high-income societies, and what they report are equivocal outcomes. In England and the United States, problems with acceptability of routine mental health assessment, together with conflict over resource allocation significantly impede both effectiveness and acceptability. The new Code has been specifically written to support better engagement between Education and Mental Health, but piloting suggests that this has not yet happened. Proactive multiagency planning within the framework of the Code, linked to explicit work on organizational transformation, could provide an effective and acceptable Schools Mental Health Service, with legal protection from subsequent deleterious policy and funding changes. Explicit recommendations for using the Code are appended. Login using your SSSFT NHS OpenAthens for full text. SSOTP - request a copy of the article from the library - www.sssft.nhs.uk/library

Major depressive disorder is a leading global cause of lifelong disability, with the greatest increase in incidence occurring in mid-to-late adolescence into young adulthood. Onset in childhood predicts poor long-term educational, social and health outcomes. Therefore, interventions to prevent depression in children and adolescents have considerable potential to reduce the global burden of depression. Prior research has suggested that selective and indicated prevention interventions targeted, respectively, at high risk or subsyndromal children and young people may outperform universal interventions that deliver to all individuals in the population.1–3 To read the full article, log in using your NHS OpenAthens details.
As part of a joint inquiry into children and young people's mental health, the Health and Education Committees found that financial pressures are restricting the provision of mental health services in schools and colleges. The next Government must review the effect of the budget reductions in the education sector, the report adds.
Compassion-focused therapy (CFT) is embedded in an evolutionary, functional analysis of psychopathology, with a focus on affiliative, caring and compassion processes. CFT has been applied in a number of adult settings, but its clinical applications in child and adolescent psychopathology and psychotherapy have not been systematically explored. This article describes the applications of CFT in paediatric populations. To read the full article, log in using your NHS OpenAthens details.
This research investigated complex relationships between parents and professionals involved in care proceedings, through detailed observations of network meetings – a practice developed in a Child and Adolescent Mental Health Service to engage with families and professionals in adversarial contexts. These take place at the border between the domain of court, often dominated by structural preoccupations, and the clinic valuing experience and relationships. There has been an abundance of studies that have employed quantitative methods to research obsessive compulsive disorder (OCD) and its neurobiology and neurochemistry. In this editorial, we discuss Morina and colleagues' meta-analysis of psychological therapies for youth with post-traumatic stress disorder (PTSD) and depression following conflict. Recent years have seen significantly more randomised controlled trial evidence addressing the needs of this population. More work is needed to understand post-traumatic depression, dissemination, timing of intervention and whether trauma-focused interventions are essential. Youth with depression are theorized to generate stress in their lives because of a complex interaction between their personal characteristics and their chronic environmental context. Using a moderated regression approach, we provided a novel test of this hypothesis by examining whether adolescent 5-factor model personality traits moderate the associations between early emotional, physical, and sexual maltreatment and life events experienced in the past 6 months. ABSTRACT FROM: Neufeld SA, Dunn VJ, Jones PB, et al. Reduction in adolescent depression after contact with mental health services: a longitudinal cohort study in the UK. Lancet Psychiatry 2017;4:120–127.
Adolescence is a critical period for the development of depression with prevalence rates rising sharply from childhood to early adulthood.1 Many adult depressive disorders have their first onset in adolescence2 with longer episode duration being the strongest predictor of future problems.3 In addition to increasing the risk of later mental health problems, adolescent depression is associated with significant educational and social impairment and is a major risk factor for suicide.1 Providing effective early interventions to shorten the duration of episodes and potentially reduce the impact on later life is therefore important. Large mental health systems are increasingly using fiscal policies to encourage the implementation of multiple evidence-based practices (EBPs). Although many implementation strategies have been identified, little is known about the types and impacts of strategies that are used by organizations within implementation as usual. This study examined organizational-level responses to a fiscally-driven, rapid, and large scale EBP implementation in children’s mental health within the Los Angeles County Department of Mental Health.
Transition to adulthood may be a period of vulnerability for health for individuals with intellectual disabilities. No large-scale studies have compared the health of individuals with and without intellectual disabilities undergoing transition. The aims of this study were (1) to compare health during transition for individuals with and without intellectual disabilities across a whole country population and (2) to establish whether transition is associated with health in the population with intellectual disabilities. To better reach youth in need of mental health and addiction (MHA) services, there is increasing emphasis on making MHA services ‘youth friendly.’ However, it is unclear what youth friendliness means on a practical level. This scoping review explores (1) how youth friendliness in mental health services is defined in the literature, (2) what characteristics make MHA services youth friendly and (3) how youth friendliness is expected to impact service use by young people.

People tend to overestimate the intensity and duration of affect (i.e., impact bias) when making predictions about their own and others' responding, termed affective and empathic forecasting, respectively. Research links impact biases to clinical symptoms of affective disorders, but little work has been done to examine how social anxiety is related to affective and empathic forecasting biases. The current investigation included two studies examining these associations in independent samples of young adults with dimensionally distributed social anxiety symptoms.. Clinical depression in children as young as age 3 has been validated, and prevalence rates are similar to the school-age disorder. Homotypic continuity between early and later childhood depression has been observed, with alterations in brain function and structure similar to those reported in depressed adults. These findings highlight the importance of identifying and treating depression as early as developmentally possible, given the relative treatment resistance and small effect sizes for treatments later in life. The authors conducted a randomized controlled trial of a dyadic parent-child psychotherapy for early childhood depression that focuses on enhancing the child’s emotional competence and emotion regulation.. Activation is a behavioral adverse event related to the use of psychotropic medication. Its high incidence in pediatrics and in childhood-onset neuropsychiatric disorders suggests it may be linked to neurodevelopment. However, previous studies have scarcely examined the role that factors relevant to developmental pharmacokinetics, such as body weight, may play in the onset of activation in children and adolescents..
