A new report from the Royal College of Nursing (RCN) and Queen’s Nursing Institute (QNI) calls for urgent investment in District Nursing, as new figures show the number of District Nurses working in the NHS has dropped by almost 43 percent in England alone in the last ten years. As a result, there are only some 4,000 District Nurses providing care for a population of around 55.8 million in England, a ratio of only one District Nurse for every 14,000 people. This compares with one GP for every 1,600 people.
A new digital portal is being introduced by the NHS and councils which allows health and social care staff to see how many vacancies there are in local care homes, saving hours of time phoning around to check availability and helping people to get the right care or return home as quickly as possible.
Councils have a major role in ensuring they get the help they need, whether it is through the services they commission or deliver. Health visitors, family nurse partnerships, early years, children’s centres, youth services and the voluntary sector all have a key role to play.
Collaboration with clinical commissioning groups to provide dedicated midwifery support and post-pregnancy contraception is also important.
The Local Government Association (LGA) and Public Health England (PHE) have developed a framework setting out how different agencies can contribute.

In 2016, the Wolverhampton Transfer of Care Project reported that avoidable discharge delays were being caused by a lack of understanding regarding intermediate care settings and difficulty in accessing these services.
They estimated that better use of an integrated service could release 318 bed days each month. They recommended an integrated model of intermediate care which would enable patients to be discharged at the point they were medically ready, and assessed either at home or in an intermediate care setting – the Discharge to Assess (D2A) model.
