This review updates previous similar papers published in JFT in 2000, 2009 and 2014. It presents evidence from meta‐analyses, systematic literature reviews, narrative literature reviews and controlled trials for the effectiveness of systemic interventions for families of children and adolescents with common mental health problems and other difficulties. In this context, systemic interventions include both family therapy and other family‐based approaches such as parent training, or parent implemented behavioural programmes. The evidence supports the effectiveness of systemic interventions either alone or as part of multimodal programmes for sleep, feeding and attachment problems in infancy; recovery from child abuse and neglect; conduct problems, emotional problems, eating disorders, somatic problems, and first episode psychosis.. Pioneering research over the past two decades has shown that memories are far more malleable than we once thought, thereby highlighting the potential for new clinical avenues for treatment of psychopathology. We first briefly review the historical foundation of memory reconsolidation—a concept that refers to hypothetical processes that occur when a memory is retrieved and restored. Then, we provide an overview of the basic research on memory reconsolidation that has been done with humans and other animals, focusing on models of fear, anxiety-related disorders, and addiction, from the perspective that they all involve disorders of memory. Like many good ideas, cognitive behavioural therapy (CBT) has gone through multiple waves of development. One view is that third-wave CBT is now rolling in, as theory and practice have moved beyond behavioural therapy, through to the incorporation of ideas from cognitive psychology, to more modern adaptations.

Psychodermatology is the interaction between skin and mind. Psychodermatological conditions are present in many patients with a mental health condition or mental health concerns. However, conditions such as dermatitis artefacta and trichotillomania are commonly misunderstood, and access to treatment may be limited. Nurses play a vital role in supporting patients and families by assessing, treating and managing such conditions, while also providing education on the condition and the treatment that is required. Mindfulness‐Based Cognitive Therapy (MBCT) is an innovative evidence‐based intervention in mental and somatic health care. Maladaptive schemas are stable relational patterns that develop through harmful childhood experiences with primary caregivers. To better concurrently address emotional and neuropsychological symptoms common in veterans with comorbid post-traumatic stress disorder (PTSD) and history of traumatic brain injury (TBI), we integrated components of compensatory cognitive training from the Cognitive Symptom Management and Rehabilitation Therapy (CogSMART) programme into cognitive processing therapy (CPT) for PTSD to create a hybrid treatment, SMART-CPT (CogSMART+CPT). Sometimes it is not clear who is pushing for transition – the child, their parents or even school authorities.With the number of referrals to the UK’s only gender identity development service (GIDS, at the Tavistock and Portman NHS Trust) increasing sharply in recent years – a pattern seemingly mirrored in other European countries and the US (anecdotally, at least — many countries don’t keep comprehensive data the way the UK does) – debate has inevitably intensified over how best to help transgender and gender nonconforming (TGNC) youth. As some expert clinicians have pointed out, there has been a tendency for commentators, campaigners and the general public to adopt an oversimplified view in which therapists are seen as fitting one of two categories: those who don’t believe their clients when they say they are trans (and who are therefore condemned by trans advocacy groups for practicing conversion therapy), and others who simply accept their clients’ statements about their gender, and who are therefore affirming or affirmative.

Psychological interventions targeted at self-harm may reduce that ongoing risk: a recent Cochrane review found that longer-term CBT (cognitive behavioural therapy) and DBT (dialectical behavioural therapy) have the most evidence for reducing future self-harm repetition but there is little evidence for more acute interventions (Hawton et al, 2016). The ideal acute psychological intervention could be delivered before discharge from an acute hospital setting and would reduce the immediate risk of future self-harm.
Current policy in the England suggests that people with intellectual disabilities should, where possible, access mainstream mental health services; this should include access to mainstream therapy services. It is likely that mainstream therapists will need training and support to work with people with intellectual disabilities. Depression and anxiety disorder prevalence rates are similar across urbanicity levels, yet rural Americans are less likely to receive mental health treatment. Barriers related to availability, accessibility, and acceptability of care in rural areas have been well documented. Cognitive–behavioral therapy (CBT) is gold-standard, evidence-based care for these mental health conditions, though its delivery in rural settings has not been systematically assessed. Conclusions: Groups delivered by videoconference are feasible and potentially can improve the accessibility of group interventions. This may be particularly useful for those who live in rural areas, have limited mobility, are socially isolated, or fear meeting new people. Outcomes are similar to in-person groups, but future research on facilitation process in videoconferencing-mediated groups and large-scale studies are required to develop the evidence base.
Cognitive behaviour therapy is recommended internationally as a treatment for psychosis (targeting symptoms such as auditory hallucinations, or “voices”). Yet mental health services are commonly unable to offer such resource‐intensive psychological interventions. Brief, symptom‐specific and less resource‐intensive therapies are being developed as one initiative to increase access. Pilot and open trials suggest that imagery-enhanced group cognitive behaviour therapy (CBT) is highly effective for social anxiety disorder (SAD). However, before being considered reliable and generalisable, the effects of the intervention need to be replicated by clinicians in a setting that is independent of the protocol developers. Cognitive-behaviour therapy (CBT) for panic disorder may consist of different combinations of several therapeutic components such as relaxation, breathing retraining, cognitive restructuring, interoceptive exposure and/or in vivo exposure. It is therefore important both theoretically and clinically to examine whether specific components of CBT or their combinations are superior to others in the treatment of panic disorder. Component network meta-analysis (NMA) is an extension of standard NMA that can be used to disentangle the treatment effects of different components included in composite interventions.
Yoga and mindfulness‐based programs are becoming increasingly popular as a supplemental intervention for children and adolescents with autism spectrum disorders (ASD). Increasing numbers of children, parents, and schools are participating in programs around the country with an enthusiasm that far exceeds the research support for their efficacy. Therapies that are safe but not effective may not cause immediate harm. Nevertheless, the misappropriation of limited time and financial resources may result in missed opportunities. Dialectical Behaviour Therapy (DBT) is an evidence-based intervention that has been included in the National Institute of Health and Care Excellence guidelines as a recommended treatment for Borderline Personality Disorder in the UK. However, implementing and sustaining evidence-based treatments in routine practice can be difficult to achieve. This study compared the survival of early and late adopters of DBT as well as teams trained via different training modes (on-site versus off-site), and explored factors that aided or hindered implementation of DBT into routine healthcare settings.

A quarter of people with psychotic conditions experience persistent auditory verbal hallucinations, despite treatment. AVATAR therapy is a new approach in which people who hear voices have a dialogue with a digital representation (avatar) of their presumed persecutor, voiced by the therapist so that the avatar responds by becoming less hostile and concedes power over the course of therapy. Of the 1.4 million new referrals for talking therapies as part of NHS England’s Increasing Access to Talking Therapies (IAPT) programme, 965,000 people began treatment, a 32,000 rise on patient numbers from the year before.
As well as increasing numbers of people getting treatment, performance statistics for 2016/17 show that waiting times are decreasing and recovery rates improving. The number of people recovering from their condition has increased on the previous 12 months, with over 50 per cent of patients making a recovery in every month of this year.

Building on support for the five-factor model (FFM) of personality disorder, the Five Factor Schizotypal Inventory (FFSI) was developed to assess maladaptive traits relevant to schizotypal personality disorder. While the development of the FFSI supports a continuity between schizotypal thinking and perception (STAP) and the FFM domain of Openness to Experience, other studies show inconsistent findings concerning the strength of this relationship. This study suggests some potential pathways leading towards maintained gains or further growth versus deterioration after naturalistic psychotherapy with experienced clinicians. This is explored in a way that transcends specific theoretical orientations. Sixteen therapies, all showing reliable improvement on Outcome Questionnaire 45.2 (OQ-45) at treatment termination but varying in later post-therapy development, were included in this study. Currently, it is unknown whether treatment outcomes derived from randomized controlled trials (RCTs) of cognitive–behavioural therapy (CBT) for problem gamblers still hold when applied to patients seen in routine practice. Thus, data from an RCT of cognitive therapy versus exposure therapy for problem gambling versus patients of a gambling help service were compared. Assessments of problem gambling severity, psychosocial impairment, and alcohol use were undertaken at baseline and post-treatment and evaluated within a counterfactual framework. Findings showed that the contrast between routine CBT for pokies and horse betting had a significant effect, indicative of a 62% lower gambling urge score if routine CBT recipients had all been horse/track betters opposed to gambling with “pokies.” However, the majority of contrasts indicated therapeutic outcomes achieved in routine CBT treatments were of equivalent robustness relative to RCT conditions. The mindfulness course led to lower distress scores after the course and during the exam term compared with students who only received the usual support. Mindfulness participants were a third less likely than other participants to have scores above a threshold commonly seen as meriting mental health support. Distress scores for the mindfulness group during exam time fell below their levels as measured at the start of the study, before exam time, whereas the students who received the standard support became increasingly stressed as the academic year progressed.

There is a significant need to improve clinical practices related to suicidal patients within contemporary mental health practice. It is argued that there is a general over-reliance on psychotropic medications and the use of inpatient psychiatric hospitalizations for suicidal risk. This reliance is puzzling given the lack of empirical support for these approaches; the evidence supporting the use of psychotropics is mixed and there are recent challenges to the routine use of inpatient care that tends not to be suicide-specific and may increase post-discharge risk. Importantly there are several psychological treatments proven effective in rigorous randomized controlled trials (RCTs). Of the replicated RCTs, dialectical behavior therapy (DBT), two forms of suicide-specific cognitive-behavioral therapy—cognitive therapy for suicide prevention (CT-SP) and brief cognitive behavioral therapy (BCBT)—and the collaborative assessment and management of suicidality (CAMS) have shown robust data for effectively treating suicidal risk. Tens of thousands of child sexual abuse (CSA) cases are reported to authorities annually. Although some of the child victims obtain psychological counseling or therapy, controversy exists about the potential consequences for the accuracy of victims’ memory of CSA, both in childhood and adulthood. Yet, delaying needed therapeutic intervention may have detrimental effects on the victims’ well-being and recovery. The proposed mechanism of action in Functional Analytic Psychotherapy (FAP) is therapist-contingent responses to client in-session behavior that parallels out-of-session problematic behavior. In theory, these responses shape adaptive behaviors that generalize into the client’s daily life (Tsai et al., 2009). Therefore, the therapist’s response repertoire must be ripe with interpersonally effective responses to model and shape client behavior.
